1. Identified as an STA 1973 Alumni; address, contact information

2. Describes, in detail, the hardship
3. Describes the ability to pay/bills/costs for item/ or needed items
4. Identifies the amount requested from the fund
5. Request is reasonable and funds are available
6. Upon review, any unanswered questions are satisfied
7. Upon review, all committee members determine and agree to the amount of disbursement
